Living Will and Health Care Power of Attorney Wednesday, April 16; 9:00 a.m.
An attorney with the Prairie State Legal Services will be at the library to help prepare an Illinois Living Will and Power of Attorney for Healthcare. This service is free and available to any person age 60 or older, under 60 years old and disabled or low income. Six ½ hour appointments are available starting at 9:00 a.m. Appointments are made per individual – a couple should make two appointments. You will need to bring the name(s), address and phone number of the person(s) you choose to act as your power of attorney. To set up an appointment, please phone the library at 697-3822, ext. 13.
